A head-on collision in Florida claimed the lives of a Chattanooga man and a Florida man Monday and left four others injured.

The crash happened Monday afternoon on Florida State Road 19 at the intersection of Lake Ravenswood Road, near the town of Altoona.

Video from the crash showed how violent the crash was, with parts strewn across the roadway and on the sides of the road.

A Lincoln sedan, with a family of four inside, was southbound and crossed into oncoming traffic and slammed into the pickup truck, driven by David Bookhardt, 63, of Chattanooga. Bookhardt died at the scene, according to the Florida Highway Patrol.

Vickie Bookhardt was also in the truck. She was listed with serious injuries and transported to Ocala Regional Hospital.

"It's a very difficult situation to investigate, not only for the family members, but to ensure we do a proper investigation for the people that passed away," said Lt. Chris Sorvillo with the Florida Highway Patrol.

"The impact of the two vehicles causes significant amount of damage, pretty significant damage to both vehicles," Sorvillo said.

The driver of the Lincoln, 33, year-old Joseph Miller died at the scene.

The other occupants of the car, 12-year-old and 8-year-old children, were listed with minor injuries from the crash. The adult passenger is listed in critical condition.
